Continuing Your Care
As we prepare to conclude private practice at the end of December 2026, our priority is helping you continue your care without interruption. This page brings together everything you’ll need: how to access and transfer your medical records, options for continuing care with our midwife Gretchen Hilt, and a list of women’s health providers in the area. If you have questions or would like help coordinating your care, our team is here to support you through the end of December.
Medical Records Access
Patients may continue to access their medical records through the patient portal through December 31, 2026. If you are transferring your care to another healthcare provider, we encourage you to request the transfer of your medical records before the clinic closes. Beginning January 1, 2027, medical records will be securely maintained for 7 years by Morgan Records Management, LLC. To request a copy of your medical records or arrange for your records to be transferred to another healthcare provider after January 1, 2027, please contact:

Continuity of care for
Gretchen Hilt, MSN, CNM, APRN
Beginning September 1, 2026, patients who wish to continue their care with midwife Gretchen Hilt, MSN, CNM, APRN, may do so through Guadalupe Regional Medical Group (GRMG), where Gretchen will be seeing patients in collaboration with Azza Abdalla, MD.
Patients who elect to establish care with Gretchen at GRMG will need to contact the practice directly to arrange appointments and coordinate the transfer or release of their medical records, as appropriate.
